Completing the Candidacy Approval Form

Following the instructions below will help you successfully complete the form and avoid the delays and problems associated with needing to resubmit it. You can view a sample completed form [PDF]. - OPEN the Candidacy Approval Form [PDF].
- SAVE AS to [YOURLASTNAME_YOURFIRSTNAME_STUDENTIDNUMBER].pdf.
- TYPE directly into the form you see displayed on your computer screen.
- SAVE TO [YOURLASTNAME_YOURFIRSTNAME_STUDENTIDNUMBER].pdf (Same file as step 2 above).
- Upload the completed Candidacy Approval form to the ANGEL Candidacy site.
Top Section
- Click on the text select tool (circled in red above). Note that as you move the mouse throughout much of the page, the cursor is in the shape of a hand. But when you move the cursor over an area where you can type, its shape changes to a vertical line.
- Last Name/First Name, etc. — Click in the area marked "Last Name" and type in your last name. Complete the rest of this section by moving to each field and clicking in the area where you wish to type.
- Plan — SLIS students taking the LIBR 289 e-Portfolio course should check "Plan b) Non-Thesis Plan". If you are writing a thesis to satisfy the culminating requirement, mark the "Plan a) Thesis (299 units req.)" option.
Degree — To add a checkmark to a checkbox, click once in the checkbox. In this example, the "MLIS" box has been checked.
Degree Major, Concentration, Competency in Written English, Change of Classification, Previous College Degree etc. — Do not fill in; leave these blank as per example.
Proposed Graduate Degree Program (Section A)
When listing your courses, follow the format in the example below. List all of the SLIS courses that you have taken or that you plan on taking. Be sure to list grades received, semester units, and the semester it was or will be completed. For future classes, leave the Grade blank, but fill in the Semester.
- Most students will list 13 classes to obtain a 42–credit degree. If you started your coursework in Fall 2008 or afterwards, you will also list Libr 203 to obtain your 43–credit degree. No more than the required amount of units needed for the program should be listed. If you have taken more than the required amount of units, do not list more than the 42 or 43 units. Be sure to list the required Core Classes of Libr 200, 202 and 204. The course grades listed on the candidacy form must result in an overall GPA of at least 3.0 (B). Students who carry an "Incomplete" grade will not be permitted to apply for candidacy. If any of the coursework that is to be applied toward your Master's Degree was completed in another department at SJSU, list it in Part C.
- List only the classes that have a grade of C or higher. Do not list any classes with a grade of C- or below. If a student gets a C or better, it will count towards the degree. If a student gets a C- or lower, that class cannot be used on the Graduate Candidacy application or counted toward the degree. Instead, the student will have to take another class in substitution to get up to 42 or 43 units.

Culminating (Section B)
In this section, list how you intend on fulfilling the culminating requirement. This can be either Libr 299 Thesis (Plan A) or Libr 289 (Course).
- Culminating: (select one only) — Check the "299 Thesis" box and indicate the units you plan to do for your Thesis and the semester in which you plan to complete the thesis. List LIBR 289 if you plan to complete the e-Portfolio option to satisfy the University's graduation requirement. Remember to list the units and semester in the columns on the right.
Courses in Other Departments (Section C)
In this section, list all courses taken or that you plan on taking in any departments outside of SLIS. Be sure to list grades received, semester units, and the semester it was or will be completed.
SJSU Extension or Transfer Resident Courses (Section D)
In this section, list any courses that have already been approved for transfer from other schools. This total should not exceed 9 units. If these courses have not been approved by Graduate Studies already, it will delay your program approval.
- Total Units — Enter the units for Part A, B, and, if necessary, C and/or D. Then add these and indicate the total number of units. This number must be 42 or 43, depending on when you started the MLIS program (if you began in Fall 2008 or later, then you need 43 units for the MLIS degree). Do not list more than the required 42 or 43 units or your Candidacy Form will not be accepted.
- Signature — Leave blank. The SLIS office will obtain the necessary signatures before sending your form to GAPE.
- Proofread carefully to be sure you have correctly completed all the requested information.
- Save to your own computer/desktop; save file as [YOURLASTNAME_YOURFIRSTNAME_STUDENTIDNUMBER].pdf.
- Log in to your ANGEL account. Click on Find a Course. Search courses for Candidacy. Enroll in this site; no PIN required. Select the Forms Drop Boxes tab and click on a drop box to see instructions for uploading your correctly named file of your Candidacy Form.
- Make sure to upload your Candidacy Form to the appropriate ANGEL drop box BY THE SLIS DEADLINE (February 6, 2009 for students planning to graduate in August 2009).
- SLIS staff will print a hardcopy of your form, review it, obtain signatures, and deliver it to the Graduate Admissions and Program Evaluation office.
Graduate Admissions and Program Evaluations (GAPE) takes at least a couple of months to properly evaluate your candidacy petition and notify you. They will notify you if your candidacy petition has been approved or denied through the mail, e-mail, and under "Other Indicators" on My SJSU after they have evaluated your candidacy petition. Make sure MySJSU has your correct and current e-mail and mailing addresses. SLIS has no information as to the status of your candidacy application; please do not contact SLIS for this information.
Course Substitution Form
Important: If you later (after you have submitted your Candidacy Approval form) take classes other than the ones listed on your form, then you must file a Request for Course Substitution in Master's Degree Program. Course substitution forms, if needed, must be submitted before submitting your Application for Award of Master's Degree form.
How to Submit:
- OPEN the Request for Course Substitution in Master's Degree Program [PDF].
- SAVE AS [YOURLASTNAME_YOURFIRSTNAME_STUDENTIDNUMBER].pdf.
- TYPE directly into the form you see displayed on your computer screen.
- SAVE TO your desktop / your computer in preparation for uploading to ANGEL.
- UPLOAD form file to the ANGEL Candidacy site. Make sure to upload into the Course Substitution form drop box.
