1. List the name and locations all the colleges and universities that you have attended, including the date attended (mm/yyyy), units completed, term type (Quarter or Semester), your major, GPA (#,##), fee status, degree, and degree date (mm/yyyy).
2. Enter the number of transferable units and answer the question if you are a registered nurse.
